The Catoosa County School System offers a supplemental insurance policy for student athletes. It's an affordable way to provide coverage or supplemental coverage.   

If you have insurance on your  student athlete  but would like additional coverage you can find the application on the school website under the athletic department. Or you can go online at www.studentinsurance-kk.com. 

If you do not have private healthcare/accident coverage on your  student athlete, please consider purchasing the policy available from the Catoosa County School System.

By signing the Lakeview Middle School Athletic Packet Parent Signature Form, you indicate that you have adequate insurance, will purchase the school supplemental insurance or accept responsibility for the financial burden for the lack thereof for your student athlete.
